What You Should Know About the New Medicaid Cuts and the Future of Hospitals

On July 4, President Trump signed the “One Big Beautiful Bill” into law. The bill delivered sweeping tax cuts but also introduced some serious reductions to federal Medicaid spending. These cuts have the potential to hit rural hospitals hard, and that could mean changes in how and where you get care.

Why Rural Hospitals Are at Risk

Many rural hospitals rely heavily on Medicaid reimbursements just to keep their doors open. The new law includes an estimated $1.02 trillion cut to Medicaid over the next decade. According to the Center for Healthcare Quality & Payment Reform, more than 700 rural hospitals nationwide could be at risk of closing. That’s nearly one-third of all rural hospitals in the United States.

Here are a few examples of how bad it could get:

  • In Kansas, up to 66 rural hospitals are already at immediate risk of closing
  • In Texas, 108 hospitals could see reduced services and 29 are at immediate risk of shutting down
  • Other states that may see big impacts include Oklahoma, Alabama, Mississippi, Missouri, and West Virginia

If closures happen, residents of rural communities may have to travel much farther for emergency or inpatient care. That’s especially challenging for seniors, people with disabilities, and anyone managing chronic health conditions.

What’s in the Law to Offset Cuts?

The bill does include $50 billion in temporary funding over six years to help rural hospitals make the transition. That money will be distributed through a new Rural Health Transformation Program starting in 2026.

The idea is to help hospitals develop long-term plans to stay open, but experts say the funding is nowhere near enough to make up for the losses. Many hospitals will still face tough choices about cutting services or closing.

What This Could Mean for You

Even if you don’t live in a rural area, you may depend on a hospital that serves a lot of Medicaid or CHIP patients. If that’s the case, you could see changes in how and when you’re able to get care in the next few years.

That might mean fewer services available locally, longer trips to see a doctor, or slower emergency response times if more hospitals close. For older adults, people with disabilities, and their caregivers, these changes can feel overwhelming. But you don’t have to just sit back and hope for the best.
